Certification
IMPORTANT INFORMATION ABOUT CERTIFICATION:
Certification is a voluntary credentialing process for providers of Cognitive Rehabilitation Therapy and:

LEVELS OF CERTIFICATION:
| 1) | Level I: |
| Persons
certified at Level I meet the criteria for designation as a "Qualified
Independent Practitioner," as defined in the American Congress of Rehabilitation
Medicine's Guidelines for Cognitive Rehabilitation. Such a practitioner
is defined as a person who has acquired appropriate credentials for the
independent practice in their discipline according to state licensing and/or
certification agencies appropriate to their profession; or in the absence
of applicable state regulations, meet the appropriate national registration
or certification requirements for the independent practice of their profession. |

| 2) | Level II: |
| Persons
certified at Level II are those who are unable to practice their professions
independently, and require supervision by a "Qualified Independent Practitioner."
The Qualified Independent Practitioner is accountable for the design, implementation,
and ongoing quality and appropriateness of evaluation and treatment services
delivered to clients. |
Certification is in effect for a five year period. To maintain certification in good standing with SCR, the therapist is required to:
| 1) | Practice
Cognitive Rehabilitation Therapy in accordance with the Code of Ethics
established by the Society for Cognitive Rehabilitation, Inc. |
| 2) | Maintain
appropriate credentials required to practice (license, other certifications,
registration). |
| 3) | Remain
in good standing with The Society for Cognitive Rehabilitation, state
regulating agencies and other professional organisations. |
| 4) | Participate
in a minimum of 50 hours of continuing education directly related to Cognitive
Rehabilitation during the 5-year certification period. Continuing education
may be in the form of classwork, seminars, conferences, workshops, or
individualised training. Proof of continuing education credits must be
submitted to SCR at the end of the 5-year certification period. |
| 5) | Apply
for re-certification. At the end of each 5 years, re-certification is
necessary. Requirements include: |
